If submitting more than 100 samples to BioTracking for testing, print and fill out these additional pages to add to your current submission form. Make sure you download and fill out the correct form:
      1) If testing with BioPRYN cattle, use the BioPRYN Cattle form
      2) If testing with BioPRYNqk, use the BioPRYNqk (Quick Test) form
      3) If testing with BioPRYN or other tests, use the BioPRYN form
